The Hospital to home package is the help to get back on your feet after discharge from hospital. We will meet with the hospital discharge co-ordinator or ward staff to ensure your discharge is co-ordinated efficiently and all your mediations are in order.  Your Support worker will ensue all follow-up appointments are scheduled accordingly.
Your carer or support worker will be able to re-orientate you to your home and put everything in the place where its more convenient for you. Your carer will also ensure you are comfortable in your environment, coordinate with the hospital consultant or outpatient clinics and work towards avoiding any unnecessary hospital admission.
